The blood pack is cool, but it doesn't do anything gameplay wise. The big plus is lack of gunpowder for those who do not enjoy that sort of thing. However, both are integrated into the same thing, whether you have shogun 2, FOTS or both, they will be listed in steam as "Total War: Shogun 2". If you like muskets and cannons, go for Fall of the Samurai. (even though there are some matchlock muskets) If you like swords, spears and bows, go for Shogun 2.
